The Vial of Life is a form that contains all your critical medical information. This includes:

● Preferred hospital

● Insurance details

● Advance directives (if applicable)

Once filled out, the form is placed in a visible, easy-to-access location in your home—often the refrigerator—and a Vial of Life decal is placed on your door or window to alert emergency personnel that it’s there.
